Polycythemia: Hematocrit Numbers And Phlebotomy

I started monthly phlebotomy for polycythemia just last spring. My physician initially prescribed sessions when my hematocrit is > 52. I argued against such a high hematocrit requirement. So, last month she dropped it to 45%, meaning I can have them whenever my hematocrit is > 45%.

What are your hematocrit boundaries for phlebotomy? Any advice?

Hi Ben!
I started out at 45 for my threshold. But now it is 42% because waiting until I reached 45% really impacted my quality of life.

I see an MPN specialist at Memorial Sloan Kettering and he said the hematacrit level should be 45 or less for men and 42 or less for women.

I have phlebotomies about every month. The original goal was to draw a unit off if my HCT was 45% or >. However, after seeing an MPN specialist at MD Anderson, he said to try to ALWAYS keep the HCT <45%. So, now, if I go in and my HCT is 43 or 44%, I still have a phlebotomy.

From what I have read, hct level goals are usually 45 for men and 42 for women. When I was first dx (Aug. 2020)my hct was 64 and I was having a phlebotomy twice a week until they got my number down to 42(which took about 2 months). I am jak2 positive. I am only on baby aspirin and Claritin for the itching. Have you had a bone marrow biopsy?
Staying hydrated and making sure you eat a little something before hand will help. I do not know if you are on Facebook, but there are several groups geared towards our rare cancer. You can also check out pvreporter.com...they have a list of mpn specialists for every state. Hope this helps.
