In people with myelofibrosis, the spleen enlarges because it's trying to compensate for the failing bone marrow. When your bone marrow can't produce blood cells properly due to fibrosis (scarring), your spleen steps in to help make blood cells - something it normally doesn't need to do in healthy people.

The spleen also grows larger because it's working overtime to remove damaged or abnormal red blood cells that the defective bone marrow produces. This dual role - trying to produce blood cells AND filtering out damaged ones - causes the spleen to become enlarged (splenomegaly).

In people without myelofibrosis, the spleen doesn't need to monitor or compensate for bone marrow production because the bone marrow is functioning normally. The spleen's main job in healthy individuals is to filter blood and support immune function, not to produce blood cells.

This is why removing the spleen (splenectomy) in MF patients who aren't getting a stem cell transplant can cause problems with low blood counts - you're taking away an organ that was trying to help produce the blood cells your bone marrow couldn't make.
