I take an over the counter iron pill once a day (most days) on an empty stomach with orange juice for the low iron. I take a baby aspirin daily for my ET as part of my watchful waiting treatment.

Low ferritin (iron stores) is something that can come up for people living with essential thrombocythemia (ET), and it's worth understanding how the two can be connected.

One key link is bleeding. ET can cause abnormal bleeding — such as nosebleeds, heavy periods, gastrointestinal bleeding, or blood in the urine. Over time, this kind of blood loss can deplete iron stores, leading to low ferritin levels.

Here are some important things to be aware of:

- Bleeding risk in ET can be paradoxical — even with high platelet counts, the platelets may not function properly, which can cause bleeding rather than clotting
- Low ferritin can cause fatigue, weakness, and brain fog — symptoms that can overlap with ET symptoms, making it tricky to identify the cause
- Iron supplementation should only be considered under a doctor's guidance, as raising iron levels could potentially stimulate platelet production, which is already a concern in ET
- Regular blood tests, including ferritin checks, are an important part of managing ET

It's really important to flag low ferritin with a hematologist, as they can help figure out the cause and the safest way to address it given the ET diagnosis.
