An important factor is that one’s predicted life expectancy has to be less than 5 years.
There is also a Risk Stratification protocol. Typically for MF, the Dynamic International Prognostic Scoring System (DIPSS) is used. Patients with intermediate-2 or high-risk scores are usually considered for BMT.

Stem cell transplants (also called bone marrow transplants) are typically considered when:
- You are younger and don't have other major health conditions
- Your disease is in earlier stages (this improves outcomes)
- You have a suitable donor match (either a sibling, close blood relative, or matched unrelated donor)
- You are healthy enough to undergo the intensive pre-transplant treatments Stem cell transplants are offered as an option because they are currently the only potential cure for MPNs. The five-year survival rate ranges from 30-70% following transplant. However, it's important to understand this treatment carries risks, including:

- Requires high doses of chemotherapy or radiation beforehand
- Possibility of graft-versus-host disease (GVHD)
- May need lifelong anti-rejection medications
- Success rates vary significantly by individual

This is why careful evaluation and discussion with your healthcare team is essential to determine if transplant is the right option for your specific situation.
