Why isn't leukemia always classified under the MPN umbrella?

Actually, some types of leukemia do fall under the MPN category! Chronic myeloid leukemia (CML) is both a type of leukemia and an MPN. It involves the bone marrow producing too many abnormal white blood cells.

The key difference is how these blood cancers behave:

MPNs (chronic conditions):
- Bone marrow makes too many blood cells
- Cells may be abnormal but still somewhat functional
- Usually progress more slowly
- Include CML, PV, ET, and MF

Acute leukemias (like AML):
- Bone marrow produces at least 20% blast cells (very immature, non-functional cells)
- Progress rapidly
- Require different, more aggressive treatment

So the classification depends on the type and maturity of the abnormal cells, not just whether the bone marrow is producing strange cells. CML is an MPN because it's chronic and the cells are more mature. When MPNs produce too many immature blast cells (20% or more), they can transform into acute leukemia.

Other leukemia-type MPNs include chronic neutrophilic leukemia (CNL) and chronic eosinophilic leukemia (CEL), which also involve overproduction of specific types of white blood cells.
