I’m a 82 year old woman. Have had PV for 3 yrs with some side effects from Hydroxy. Recently it’s affected my platelets which have plummeted. Doc wants to change med to Interferon which has me freaked since side effects are horrific !! I would appreciate any info/advice on taking this drug. Thank you so much. Elaine Givner

What should you know about switching from hydroxyurea to interferon for polycythemia vera when experiencing platelet issues?

It's understandable to feel concerned about changing medications, especially when you've heard about potential side effects. The good news is that newer formulations of interferon have significantly improved compared to older versions.

Older interferons were known for severe flu-like symptoms and causing blood cell counts to drop too low. However, newer formulations are proving safer and more effective, with ongoing research to optimize these medications.

Interferon works differently than hydroxyurea - it triggers your immune system to break down unwanted cells rather than affecting all blood cell types. This targeted approach may help with your platelet fluctuations.

Common experiences with interferon include:
* It's given as an injection (can be self-administered at home)
* Side effects are generally milder with newer versions
* It's often recommended as a first-line treatment, particularly for younger patients
* Some myMPNteam members have successfully switched to newer interferons like Besremi when hydroxyurea wasn't working well

One member shared that after hydroxyurea caused their iron to drop and affected their quality of life, they switched to Besremi, a newer interferon formulation.

Since you're experiencing platelet drops with hydroxyurea, your doctor likely believes interferon could provide better control. It's worth having a detailed conversation with your hematologist about which specific interferon formulation they're recommending and what side effects to realistically expect with that particular version.
