I don't understand why a BMB would be necessary with such a low platelet count. It would seem that a watch and wait strategy would be appropriate. I have had ET for 4 years with platelets currently at 860. I am 74, I have refused chemo and have not had a BMB. Somehow my hematologist determined I was Jak 2 from blood test. I have no symptoms. A platelet count in the 400K range should not cause you any health problems unless you have severe comorbidities. Mayby a second opinion is waranted.

Miranda, great to hear the count went down. Maybe you could just be monitored, CBC, every three months. What is your Dr./hematologist saying is necessary monitoring. A specialist may order the MPN mutation panel but some local drs may not. I really hope your platelets stay in the 400,000's. ✌️

So maybe I could request more testing to see anything else showing before a bmb

Hi, have you had more than one CBC with the result of high platelets? Did I miss this information... If only one high count, there could be another reason other than an MPN causing the high count. When I had the first high count, which was in the low 500,000's, my PCP did an immediate repeat CBC. My count was slightly higher. If you've had several counts all increases and even slight differences, say up or down but still high) I might hold off on the BMB but request genetic testing to see if you have a gene mutation that is common with MPN's, an MPN panel at the least. You would still need a BMB for diagnostic purposes, but knowing what I know, I would want the genetic testing done first. A hematologist would/should order both if you've had more than one high count. There are NCCN Guidelines that state the diagnosis process for ET. All my best.
