Yes, it is safe. Hydrea has been around for decades and is the usual first line of defense for MPNs like PCV. It's cheap and readily available. I hope I never have to get off of it, because it's worked well for me for the past 10 months, since I was diagnosed.
I know someone who has been on Hu for 19 years and she has been fine albeit at the low dose just one capsule a day. I don't get on with Hu but that's another story. I am reducing my dose and having venesections to take up the slack. Hct at 45 is good for most people...lower than mine is!
I have been on 500 mg of hu for three years with occasional mouth sores and ringing in my ears. Platelets 250 but hemocrit almost 45. May have to start phebotomies as I do not want to take a higher dose of hu.
